LIFE Counseling & Assessment Center also provides accurate and timely implementation of the substance abuse evaluation, makes necessary treatment recommendations, determinacy of compliance or non-compliance, and follow-up directions.
With frequent updates and additions to the law you can feel confident when you come to LIFE Counseling & Assessment that you have a qualified professional and are in full compliance with all DOT/SAP rules & regulations.

For Juniors $2,000, plus $50 monthly fee.
Seniors $2,500, plus $100 monthly fee Discount for Juniors and Senior who are currently in our program.
Services may be covered in full or in part by your health insurance or employee benefit plan.
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the Full Cost of the treatment as booked.
