Home Base Collaborative Family Counseling (Home Base) strives to help members of the New Hampshire community become whole. We hope to join clients in the process of transformation as they define their future and find their voice. Our Outpatient Programs offer a comprehensive list of services focused around promoting growth and fostering change through empathy, acceptance, and compassion.
Our community-based programming through DCYF & DJJS strives to: maintain parent-child connections; foster hope; increase safety and stability in families; remove barriers and stigmas to accessing Mental Health Treatment and Clinical Case Management; believe in parents' ability to change, and provide them the necessary skills; and, to help parents, children, and families heal.
Our school-based programming helps students develop positive self-esteem, prosocial skills, age appropriate emotional and behavioral skills so they can experience success at home and school. If a client is physically disabled and any of our office locations are unable to provide the proper accommodations to assist that client (i.e handicap entrance/bathroom/etc), we will conduct the scheduled appointment in the client's home or other location agreed upon by the agency and client at no additional cost to the client.

Home Base Collaborative's School to Home (S2H) program strives to help students, families, and schools in working cooperatively to learn, grow, and succeed in the community.
S2H works with children and youth whose classroom learning is affected by social, emotional, and behavioral challenges.
Our team utilizes a strengths-based approach to build healthy connections between school culture and the home environment.
We focus on reinforcing student progress by removing barriers that hinder educational, social, emotional, and developmental growth.
