As a provider of adult foster care services and mental health counseling in Mattapan, Massachusetts, we are committed to delivering quality services and programs to the communities we serve. Services are provided in a manner tailored to each individual in order to manage their illness, set individualized goals to be accomplished, and to develop skills in an effort to promote social, emotional, economic, healthy living, and behavioral health within families and communities.

Being a provider of mental health counseling in Mattapan, Massachusetts, we deliver holistic patient-centered services, meaning we customize a treatment plan based on your specific needs.
This will be done through a comprehensive assessment.
We work with you, your family, and the other professionals handling your care so that you will be able to maximize your treatment.
The Clinician provides counseling therapy, crisis intervention, and/or case management services in an in-home and/or community setting.
The Clinician cares for the needs of the specific patient population and provides appropriate direct patient care through assessment, diagnosis, and treatment of specific conditions.
Provides counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position; prepares treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and urgent services as required.
Your medical provider must authorize your need for transportation by completing a Prescription for Transportation (PT-1) form for community-based services or a Medical Necessity Form (MNF) for institutionally based services.
Transportation must be to and from a MassHealth provider for a MassHealthreimbursable service.
