I am a Licensed Professional Counselor, a National Certified Counselor, and a Board Certified Coach with several years of experience counseling young adults and adults. Payment is due at time of service and is payable by cash, check or debit/credit card (an additional processing fee of $4.80 will be added for debit/credit card transactions).
While I do not accept any other insurance coverage, I can provide you with a statement ("Superbill"), which you may then submit to your insurance carrier for reimbursement.
Since I am not on any preferred-provider or managed-care lists, I am considered an out-of-network provider.
