Conveniently located on Glen Avenue next to the Metro Centre in Peoria, we accept most major insurance plans, and are contracted providers with several Employee Assistance Programs including: CMFI, Army Corps of Engineers and Chestnut Health Systems (which includes several companies - Caterpillar, OSF, Unity Point Health, etc). Please see your Human Relations department to determine your benefits or with any insurance or payment questions.
